Service Application Form

At the end of the application process you will need to print the generated application form and provide a Birth Certificate, Passport, OR Drivers Licence as Evidence of Identity. Australian applicants have the added option of providing a Medicare Card.

After you have completed the registration process your application will need to be approved (usually within 2 business days).

The application process takes 10-15 minutes to complete. If the webpage remains inactive for approximately 30 minutes, your application will timeout and you will need to start the process again. If you have specified an EDI or Messaging Address and your session times out, that EDI or Messaging Address will not be available for use for 24 hours.
